I sold company stock that was given to me as part of an incentive compensation plan. do I use the date it was given to me or the date that it vested as the acquired date?

If it was stock options, you would use the date you exercised the options.  If it was restricted stock or RSUs or similar it would be the date it vested.  The date that triggered a taxable event is the date you use.
