When asked to enter rent and total expenses for entire home am I entering in a monthly or yearly number?

Yearly. You would be entering the total rent income received for all of 2019 and the total expense for 2019 in each category of expenses. This is assuming that the property was rented for the entire year. If it was rented less than the full year, then the expenses may need to be prorated to cover only the period the property was rented.
