Enter a worthless security as a sale on the last day of the year  for $0 under

Federal On left

Wages & Income

Then scroll down to Investment Income,

Then Stocks, Mutual Funds, Bonds, Other - click Start or Update

 

Or type Worthless Stock in the search box

What form do I need from my bank to report tanked/bankrupt stocks?

it is a capital loss. it will first show up on form 8949 which will flow to schedule D.
