How do you determine cost basis for stock distribution

I have received and sold stock from my company at various times from 2013 to 2016. I sold it in 2016. I have the amounts I received at each time, and the fair market value. Do I need to multiply the FMV x amount of stock received at each various time to get the total needed for my taxes?

Yes, in your case, that's exactly how it's done.

If there are any reinvested dividends, you need to add those to the FMV too.
