How do I determine who the bank or brokerage is on the 1099b form?

I do not see a bank or brokerage name on my 1099b form. The payer's name is just the State of NY

Did you get the money thru one of those state "lost money" places ?   If not then you must know who your money is invested in ... and for stock sales you don't need that info just the stock you sold.
