You can count as medical expenses the amount you actually pay out-of-pocket in 2016, not the amounts billed or amounts reimbursed by insurance.

Neither. You can deduct as medical expenses only what you have actually paid out of your pocket. What was billed doesn't matter, what your portion after insurance doesn't matter. All that matters is what you actually paid out of your pocket. You can deduct that in the year that you pay it regardless of when the service was provided.
