"Enter bleow any premiums you paid for unsubsidized dental, vision, or health insurance." Is this a total for the year or a monthly cost?

I don't know which screen you are referring to, but it is almost certainly asking for the annual amount.

Be sure that you enter only any after-tax amounts paid. Most employer health insurance is subsidized by the employer and would not go here. Ask your employer to be sure.
