My college's 1098-T showed only $5 because the CA Board of Governor's Waiver cover the 655. Do I say they didn't report the full amount or use their statement?

Use their statement.  Tuition remission/waivers are not treated as tuition actually paid. 

 

That answer assumes the $5 was in box 1 of the 1098-T and the  $655 was not in box 5 of the 1098-T.
