Question

In box 12 next to the D is a 19 then a space and a dollar amount xxxx.xx, was wondering if the 19 means anything?

I don't think it has any significance for your tax return. Box 12 should have a code (such as "D") and a $ amount. I would just ignore it.

Ignore the 19. That merely signifies the year that the code D (401(k) plan contribution) was for. Just enter the D and amount.
