If we purchased hearing aids in 2018, but will pay them off over 18 months, do I claim the whole purchase for 2018, or just the monthly payments made in 2018?

You can only deduct the total monthly portion you pay in 2018. Per IRS, you are only allowed the medical deduction for the portion of expenses you have paid in the year.
