Question

If I filed form 4852 here on TurboTax do I have to personally mail the form to the Irs? Or does TurboTax send it to the Irs?

It should have sent with the mailed federal tax return.  If the Form 4852 was completed with the federal tax return, the tax return should not have been able to be e-filed. 

If it was e-filed then you will need to mail the 4852 to the address for a Form 1040.
