An incidental death benefit would generally be reported on a code 4 Form 1099-R that you would receive near the end of January and you'll enter that form into TurboTax under Wages & Income -> Retirement Plans and Social Security -> IRA, 401(k), Pension Plan Withdrawals (1099-R).
