Misplaced the 1099-G form; how can I obtain the info to complete taxes?

It depends on what type of income is reported on your 1099G Form. If this is a state income tax refund, you should be able to access and print this form through the state department of revenue website.
If this is unemployment compensation, you should be able to access and print this form through the website that you used to file your unemployment claim.
If this is paid family leave, you would need to contact the agency or company that you received the money from in order to get a copy of this form.

I'm assuming your state is Illinois. If so, the IDES page here will allow you to log in/create an account and print your 1099-G.
