Inherited 1099r use the other persons birthdate? Turbo only gives option for ... took all as an RMD. is this correct?

also confused as to what the taxable amount is....undetermined is checked and I select that but turbo still asks the taxable amount.  Is this all ok?

Yes, you do use the deceased person's birthdate, not your own. And Yes - you can say that all is RMD if you took the full distribution. And yes, do report the form exactly as it appears, if the "undetermined" box is checked, you check it as well.  And the program will ask about the taxable amount, most likely the entire amount is taxable.
