Did a rollover from 401k to another 401K. Got a 1099-R from both. Do I report both?

There are two 1099-R forms.  Do I report both?

Yes, enter both 1099-Rs, paying close attention to the box 7 distribution codes.

A 1099-R shouldn't be generated be the receiving 401k, unless those funds were withdrawn or rolled over again.
