I rolled over my 401K into a traditional IRA in 2016, but I didn't receive a 1099-R. Do I need to enter something somewhere in my taxes?

Retirement tax questions
Call the 401(k) custodian for a copy of the 1099-R. They were required to send it to you by Jan 31, 2017. You need the form to report it on your 20916 tax return.
