If form doesn't say send with 1040 document do I make copy of form anyway and attach to 1040?

I have a 1099-G State Louisiana showing how much I receive in 2017 refund. The form doesn't show to keep or send with 1040 but I had to report the amount on the 1040 when asked how much received last year. Do I need to make copy and send in?

No you don’t send that in.  Only copies of all W2s and any 1099 that show withholding on them.  Usually like a 1099R.
