Where do I enter a direct rollover from 401k to a new opened IRA account? I did not receive any 1099 form.

The plan administrator of a 401(k) must always issue a Form 1099-R for a direct rollover from a 401(k) to an IRA.

This would be true if this was a trustee-to-trustee transfer of one IRA to another IRA. However, the plan administrator of a 401(k) must *always* issue a Form 1099-R for a direct rollover from a 401(k) to an IRA. Contact the 401(k) plan administrator to obtain the missing Form 1099-R.
