i have figured it out
1. Form 1099-R revisit
2. click edit next to the amount of 5500 imported from fidelity
3. next next until you reach the screen "what did you do with the money from National Financial..."
4. select "i moved the money to other retirement account" and "I converted all this money to Roth IRA Account"
that will fix the form 8606 Part II 16 and 17 to say 5500
