I have a 1099-R for a 401K do I attach copy 2 for the state taxes with held and copy B for the Federal.
I am not retired but is of age where i can take from my 401k and not have to rollover so how do I attach the 1099-R to both the federal and state, the taxes are already done I just need to know where to attach the 1099-R for mailing.

Retirement tax questions
Federal Return:
If federal tax was withheld, then you will attach copy b of Form 1099-R along with Forms W-2, W-2G and 2439 to the front of Form 1040.

State Return:
Each state has different rules for what should be included when mailing your return. You will need to check your state's website. However, if you are required to send a copy of Form 1099-R then you will send copy 2.
