401k rollover to qual plan. completed the 1099 info TT asks Where is this distr from. Choices seem irrelevant. But "none of the above" results in qual disaster questions.

Ran out of characters above. I rolled over a 401k into a qualified plan. TT asks "where is this distribution from?". The choices seem irrelevant; however if I select not applicable the system seems to assume that I had a qualified disaster. 

I'm unclear what I need to do. Thank you.
