Is there a form that shows the details of funds rolled over from one institution to another?

I rolled a 457(b) over to an IRA (not a Roth) at a different institution. The 1099-R says $0 is a non-taxable distribution in box 2a. The amount of the rollover shows up in the 1040 as income but is not taxed. That's good but should the rollover show up as income at all?  Should TurboTax have opened a form showing the rollover?

No, there's no form to show your retirement plan rollover.  Based on what you stated, you reported your rollover correctly.  And yes, the non-taxable amount of your rollover is supposed to show up on your Form 1040.

The rollover should appear on Form 1040 as nontaxable income with the the amount rolled over included on line 5a but excluded from line 5b and with the word ROLLOVER next to this line.
