I had an HSA through my employer for 2016. They contributed $ ($125/quarter) I did not contribute anything. My "plan type" on my medical insurance says "PPO". How do I answer the HDHP question?

It sounds like you should answer 'no' you didn't have a HDHP. You had a PPO type insurance plan.
