If you have contributions to an HSA or distributions from an HSA in 2018, then the IRS requires you to add form 8889 to your return.

Form 8889 is not supported by the Free Edition, so, yes, you would have to upgrade (normally to Deluxe).

NOTE: the first sentence means that just having an HSA is not the requirement for the 8889, but having some activity in the HSA makes the form required. This allows a few people who have an HSA but no activity to skip reporting it on the 8889 in a year with no activity.
