Just enter the information as you have it on the form.  You don't have to enter the EIN for the company.

@Deebaby80   You need an EIN number for what?  For your own business or from someone else?  If you are a sole proprietor or Single Member LLC you don't need an EIN unless you pay employees on a W2.  Otherwise you can use your ssn.
