Yes, leave the incorrect address on your 1099-SA. The IRS will not use this address for anything other than comparing it to the copy of your 1099-SA that they received (and probably not even then).
The address that has to be correct is the one at the top of your 1040 (you entered this in your My Info or Personal Info section).
