If you took a distribution from your Health Savings Account (HSA) or Medical Savings Account (MSA), you should have received a Form 1099-SA. Contact the financial institution that manages the account and request a copy of the form. If a Form 1099-SA was issued and you don't report it, the distribution will be treated as taxable.
