where to add an entry on AZ State tax form
Form 140: Line 13 - Non-Arizona Municipal interest has been entered. This entry requires a schedule listing the payors and the amount received from each payor.
Non-AZ Muni interest $4.
I received the above message when finishing up my AZ state taxes. Where do I find this schedule? I was offered no schedule, nor given an indication on where to find it.

State tax filing
Go back to your Arizona interview. Near the beginning, you will see a screen with the heading "Arizona Municipal Interest". Enter the dollar amount in the box labelled "Federal exempt non-Arizona Municipal Interest Income".
Hit Continue.
The next screen show present a several of blank boxes into which you can enter your "statement".
