This is starting to get frustrating!  I start to print my 2018 US and IL tax report filings and it appear to be moving along until a screen came up on the US filings that stated I owed $1500+ and followed with two brackets -

1 - pay automatic payments on line of $31 a month and how much was going to be sent with now … ? … I am not send another copy on line.  I am trying to download a copy of each, US and IL, to mail to an address that was sent with the requests for copies.

2 - US has drafted five amounts of $100 each month already, so, I don't believe I need to answer those questions about new payments to be made.

But, US did state the the Form 1040/A/EZ is blank, illegible, missing, or damaged and it cannot be processed, so I was to see that the form was actually completed with my filed pages and if not I need it to be completed and signed to send to an office in Fresno, CA.

Okay, please provide your help to get these things done.

I agree with Critter.  You need a conversation with customer support.  Your post is very confusing and I think you are very confused. If you are working on a 2018 return there are not any Form 1040EZ or Form 1040A tax forms for 2018.  For 2018 there is only Form 1040.

It sounds like perhaps you have an existing installment payment plan going for some past tax year, and have it confused with 2018's return somehow,  You need a real time phone conversation to sort it out.
