Where can I find my form for my amendment? Do I have to mail the forms to both NY state and Fed? Where do I mail them to?

Using the instructions below to amend your tax returns (federal and state) you will find all the instructions including forms to file and print as well as the mailing addresses. Form 1040X on the federal return and any forms that were changed because of the additional income, deduction and/or credit. New York amendment form is IT-201X
