Yes.  Each year is separate.  You need to file the extension each year.

The grant of extension of time is automatic. There is no proof of filing that request.

IRS will contact you if your extension is denied.

If you made a payment with your request, as expected,

your account statement on the IRS website will show a "payment" but it is not marked form 4868 or extension.

Only the date of payment is shown.
