Anytime you have an address change, you should fill out IRS Form 8822. This form is not filed with your return-mail separately. You can also go to a local office or phone in a change of address after they verify your identity.

You send the Form 8822 to the address based on your state of residence. The mailing address is on page 2 of the Form 8822 - https://www.irs.gov/pub/irs-pdf/f8822.pdf
