I see that the page "shows" those boxes, but I don't see anything that asks for the data.
There is also a note at the top : "
"Dividends and gains (boxes 1a-2d) - Leave blank if empty on your form."
I'd add to that, to leave any of the boxes blank if your 1099-DIV form shows a 0.00 in that particular box too
