I'm doing my daughter's taxes because she is 16 so I was confused about whether to put her name or my name on the consent form

ok, thank you. And on the very first page, before you get to the consent form, do I put her name or my name.

You should prepare the return as if you were your daughter and she was preparing her own return.

Use her name. However, if this is her first tax return, she may need to file by mail.
