My wife changed her last name and on one of her W2's has her Maiden name. Do I file normally or is there extra paperwork needed

If your wife has changed her name with the Social Security Administration and received a new Social Security card, she must use her new name on her return (or the joint return).

If one of her form W-2 shows her maiden name, this is not important. You just report the W-2 as hers on the tax return as the IRS can identify her by her SSN.
