I suggest going back on TurboTax and completing the Health Insurance section. Form 8965 is generated when you answer certain questions in the Health Insurance section. Then, you can print Form 8965 and mail it to IRS.
TurboTax will automatically fill our Form 8965 (including the exemption code) if anybody on your return didn't have health insurance coverage for the entire year.
