Since we moved December 10, 2016 and there is only a few weeks of 2016 that I would be using the new home office, do I need to enter two home offices?

If you began using an office in the new home as soon as you moved in, then you should enter a second home office.

You could wait until the 2017 tax return to enter the information for the new home office if you did not start using it immediately when you moved or if you want to forgo claiming any home office expenses past the date of your move.
