It’s asking me to enter number from box 12 for HSA info but there are 4 box 12 and I’m not sure which is the correct one.

HSA contributions are reported with code W in box 12 of your W-2. Enter all four box-12 items in the four box-12 boxes of TurboTax's W-2 form, nowhere else in TurboTax.
