Question

I purchased a piano from charity that is qualified for federal and state deduction (found in the receipt), but I don't know how to add it on my tax return.

Please explain how you think an item you purchased can be entered as a charitable donation.   You got something for the money you paid.     Did you pay way more than the value of the item?
