Did you make a withdrawal from your tax deferred retirement account in 2021, 2022 or 2023 due to a federally declared disaster?  If not, answer NO.

If you have been through a disaster and need to fill out the form, you should complete the form.  You may have suffered losses or taken a distribution. You can locate your FEMA/ declared disaster information using this IRS tool. See What if I have property that was lost or damaged (a casualty loss)?

If you have not been through a disaster and not taken a distribution from a retirement plan, delete the form. See How do I view and delete forms in TTO?
