I keep trying to research the 1099-hc form and im not learning a lot. Do i have to file this with my return or not?

Yes, you will use the information from the 1099-HC to fill out Massachusetts Schedule HC when you file electronically. The is not included anywhere on your Federal Return only your state return.

If you are filing by paper (printing and mailing your return) you will need to attach the 1099 HC to your return.
