I have S125 typed into box 14- would I put this under "Box 14 - Description" or "Box 14 - Category"?

I have S125 typed into box 14- would I put this under "Box 14 - Description" or "Box 14 - Category"?

Type "S125" into the description.

For tax category, if it's one of the NY 125's listed, select the appropriate one. If not, and none of the others are relevant, simply select "Other."
