You will need to report these line items separately. In the W-2 entry section, there should be a link that says +Add another row just under box 12. Click it, and you can enter each line item separately.

State of Alabama board of education employee. How should I enter box 14 when it gives a code of 414, CAF and DUE and all 3 codes have amounts?

Box 14 on Form W-2 is a 'catch all' box for employers to leave notes and rarely affects your tax return.  If you don't find a code in the dropdown list to match, you can choose 'Other'.

 

Specifically, 414 (or 414H) refers to pre-tax retirement contributions, CAF often refers to a Cafeteria Plan, and DUE is usually a non-taxable union or professional fee.
