Yes, to be able to claim the Child and Dependent Care Tax benefits/Credit, you must provide the Caregiver's Tax ID and other information.  They should gladly provide this to you, if not you may still be able to claim, but it will take longer.

By federal law a daycare provider is required to provide you their SSN or EIN, along with the total amount you paid them in the tax year. So it should be a simple matter of you contacting the provider, proving you are a client of theirs, and asking for the information.
