Form 1041-V is a payment voucher that you mail with an estate or trust income tax return when there is a payment due for the income tax liability of an estate or trust.

1041-V: https://www.irs.gov/pub/irs-access/f1041v_accessible.pdf

You do not enter the voucher anywhere on the income tax return. The payment amount that appears on the voucher is typically from Line 28 on Form 1041.
