My form 1099G only has lines 1-4 but im being asked about lines 10a,10b, and 11. Do i just leave those blank?

Yes, the lines are providing state information.  if you don't have any dollar amounts or other value in lines 10 and 11 of your Form 1099-G, you can leave it blank.

Hello, thanks for your reply. I realized I was not specific enough. It is not that those lines are blank, they simply arent there. My form only has lines 1-4 and no other lines, filled or empty.

If the issuer didn't provide any information which could be on the lines, the answer to your question is the same, leave it blank. It is quite common, the Forms 1099-G are not always contain all the boxes/lines.
