Line 13 Form 1040-ES

Thanks for your time.
 
I have a question about 1040-ES that I'm using to estimate quarterly taxes.
 
For context, I have a W-2 job and also freelance. My W-2 employer withholds Federal, Social Security, Medicare, and State taxes from my paychecks.
 
On Line 13 of the 1040-ES, where it asks about income tax withheld:
 
1) Do I include federal, payroll taxes and state/local taxes my W-2 employer withholds form my paychecks?
 
2) If I don't include all of those things on Line 13 of the 1040-ES, which of those things do I include?

Use the amount in box 2 of your W-2.
