Indiana schedule 7 lists filer and spouse died in 2017. Not as entered in personal info

Personal info input as widow not having died.  Spouse died before 2015.  Indiana Schedule 7, line 5 says both died in 2017.  How can we fix schedule 7?

On Indiana Schedule 7, the year of death is "preprinted" on the form. It does not indicate the actual date of death unless the date and the month are also printed.
