If I teach lessons at a music store, do I need some kind of tax form from them?

Yes, they should be sending you a W-2, if you were an employee or a 1099-MISC if you were an independent contractor. These have to be mailed out to you by January 31st.

If they don't send you a 1099-MISC, you still have to report it as self-employment income.
