I am the owner of my LLC and the only employee so I do not have a 1099 form. How do I proceed?
Unless you have elected to be taxed as corporation or Sub S the LLC is taxed as a "disregarded entity". That means you file as self employed. No 1099 or W-2 needed.

To expand on the response by @ee-ea, if your LLC is disregarded, you will file this information on a Schedule C.
